Pre-Travel Assessment

To start this process:
Contact us through our triage form
- Select "I have an admin or routine care request"
- Enter your details
- Select "Routine care appointment"
- From the drop down list, select "Pre travel appointment"
Our admin team will see this and send you a travel questionnaire to complete via SMS
