If you would like to book an NHS Health Check and you are aged between 40 and 74 and do not have any pre-exiting medical conditions.

1. Please contact the surgery on 020 8780 5650 to book an appointment with our Health Care Assistant.
2. Once your appointment is booked please complete our Health Check Questionnaire
